AB0575 FACTORS ASSOCIATED WITH PSORIATIC ARTHRITIS ACTIVITY IN CLINICAL PRACTICE

نویسندگان

چکیده

Background: Psoriatic Arthritis (PsA) is a multifaceted disease with various clinical domains. A number of composite indices has been developed recently [1, 2]. DAPSA ( D isease ctivity in PS oriatic rthritis) the valid instrument for assessment PsA activity focused on peripheral arthritis. But other factors can influence and haven’t studied yet. Objectives: to identify that are associated based data from RU-PsART cohort. Methods: 117 (M/F=63(54%)/54(46%)) patients (pts) active fulfilling CASPAR criteria mean age 44.3 ± 10.9 yrs, 37.8±21.4, duration 73.4±78.4 mo, PsO 213.4±152.5 were included. Data was collected 43 rheumatology centers Russian Federation. All pts underwent standard examination at enrollment. DAPSA, ESR (mm/h), CRP (mg/L), dactylitis, enthesitis by LEI plus Plantar Fascia, BSA (%), HAQ-DI fatigue Fatigue Scale (V.4) FACIT (Functional Assessment Chronic Illness Therapy), BMI (kg/m 2 ) evaluated. > 28 indicate high activity. Multi-dimensional step-by-step discriminant analysis used group features DAPSA. M±SD, %, OR 95% (CI) performed. Results: The following proved be most informative: (p=0.739), tenderness Fascia (p=0.003), (p=0.0004), (p=0.092), >2 yrs (p=0.013), BSA>3% (p=0.021), Nail (p=0.22). area under ROC-curve (AUC) 0.768, CI (0.624-0.913). Sensitivity/ Specificity model 77.0%/ 69% accordingly (Figure 1). Figure 1 ROC Sensitivity model. Conclusion: Weight, enthesitis, skin nail severity, as well articular symptoms. New combination these should validated practice. References: [1]Helliwell P, et al. development candidate responder psoriatic arthritis (GRACE project). Residual disease activity and treatment adjustments in psoriatic arthritis in current clinical practice

BACKGROUND With expanding therapeutic possibilities for treatment of psoriatic arthritis (PsA) it will be increasingly important to determine residual disease and define when to adjust treatment. The rationale behind treatment decisions in current daily clinical practice and the relationship with residual disease activity has not been investigated. The aim of this study was to assess current cl...

Comparing bone mineral density in rheumatoid arthritis and psoriatic arthritis

Background: Changes in patients’ bone mineral density (BMD) is one of the problems in patients with rheumatoid arthritis, which can be due to the use of corticosteroid drugs to reduce patients' symptoms or due to the aging process in patients. In this study, we decided to compare BMD in patients with psoriatic arthritis and rheumatoid arthritis. Methods: This study as a cross-sectional descrip...

Clinical presentation of psoriatic arthritis.

Psoriatic arthritis is a spondyloarthropathy which occurs in patients with skin and/or nail psoriasis. Basing its characterization on morphological purposes, several types of arthritis have been described. Alternatively, we propose a simplified classification into three subsets, focusing on the levels of expression of cutaneous and articular elements which devise this syndrome. The first is est...

Achilles enthesitis defined by ultrasound is not associated with clinical enthesitis in patients with psoriatic arthritis

OBJECTIVE To compare clinical and ultrasonographic (US) evaluation of Achilles enthesitis in patients with psoriatic arthritis (PsA). METHODS The Achilles insertion of outpatients with PsA was examined by clinical assessment of tenderness and US evaluation of (1) inflammatory activity (defined as the presence of power Doppler signal, tendon thickening and/or hypoechogenicity) and (2) structur...

Assessment of disease activity in psoriatic arthritis.

Psoriatic arthritis (PsA) historically has been assessed according to disease activity measures borrowed from rheumatoid arthritis. However, more disease activity measures specific to PsA have been developed. This development is appropriate, as the disease is not confined to the joints but has multiple manifestations, in addition to skin and joints. Assessments of disease activity are unique to...
